This page begins the formal referral partnership process. Before any individual is sent, directed, or referred to Holy Ground, an approved Memorandum of Understanding must be on file with our organization. That requirement is not a barrier. It is how we protect the people we both serve.

Complete this application with accuracy. Review the process below carefully. If approved, Holy Ground will issue an MOU and assign your organization a unique Referral Partner Code for all future referrals.

Every individual referred to Holy Ground goes through our intake, eligibility review, documentation, and case management process.
