Zoom Appointment
A Zoom consultation appointment will be scheduled and sent to your email.

Identification Upload
Before your consultation, upload your valid identification to the email address provided in your confirmation email.

Interview & Services
You will meet with an agent or the director via Zoom for an interview to discuss available resources, including enrollment in the Mind 2Thrive program.
